INFORMATION COFFEY INSURANCE AGENCY RECEIVES ABOUT YOU

Information disclosed on your application and other forms;

Information furnished by third parties including consumer credit history, motor vehicle records and medical records; and

Information provided to us by you or your representatives upon filing a claim or as part of any other transaction involving your policy.

INFORMATION WE MAY DISCLOSE TO THIRD PARTIES

We do not disclose to any third party nonpublic personal financial information about our customers, former customers or consumers without their consent, except as permitted by law.

THIRD PARTIES TO WHOM WE DISCLOSE INFORMATION

Coffey Insurance Agency may disclose nonpublic personal information to the following types of third parties:

  • Third party administrators and companies assisting Coffey Insurance Agency with routine business practices including application processing, claims handling and policy change requests;
  • Companies that help Coffey Insurance Agency and the insurance industry monitor and prevent insurance fraud;
  • Law enforcement agencies, state insurance regulators and any other entity to whom we are required by law to disclose such information; and
  • Your Coffey Insurance agent, who markets our affiliated companies’ products to our policyholders (this would consist of your name, address and policy number available to your agent).

We maintain physical, electronic and procedural safeguards that comply with federal regulations to guard your nonpublic personal information. Third parties with whom we share nonpublic personal information are required to maintain the confidentiality of such information.
